Case Management Tech (CMT), COBRA Case Management

Description: Assist the COBRA Case Management Team in providing support services to clients and client families. Assist the case managers in completing individualized treatment plans, implementing service plans, recording, updating, maintaining and submitting case record reports and other required recording forms or reports and keeping information confidential. The CMT participates, as a member of the Case Management Team, in planning and coordinating case conferences, assessing the service needs of each client, and formulating and implementing case closing plans.

Assist clients in accessing benefits and services, including advocacy and referrals to both internal service offered by Bailey House and supportive services offered by other local community organizations. The CMT will attend and actively participate in team meetings, case conferences, and other interdisciplinary forums that focus on planning, problem solving and providing effective services to meet client/family needs. The CMT is responsible for all appropriate documentation including recording and updating case notes for the records and billing activity logs.

Requirements: Must possess an A.A. or related degree plus a minimum of two years community relations or other direct client experience. Experience in COBRA a plus. Experience with individuals living with HIV/AIDS a plus. Experience with charting and documenting progress of clients preferred. Excellent communication skills a must. Must possess the ability to set boundaries and limits with clients. Ability to travel within the Metro NYC area a must. Knowledge of AIDS/HIV resources and benefits in the New York City Metro area a plus. Bilingual (English-Spanish) required.
